Join to apply for the FT Hospice Spiritual Counselor role at Coastal Hospice 1 week ago Be among the first 25 applicants Join to apply for the FT Hospice Spiritual Counselor role at Coastal Hospice Role Promote consistent, high quality patient care by functioning within Coastal Hospice IDT model in meeting patient/family goals and maintaining effective relationships with contracted facilities Provide assessment, planning, intervention and ongoing evaluation of spiritual needs to meet assigned patient and family goals by: Attending and actively participating in scheduled meetings of the designated IDT Collaborating with members of the IDT on ways to support the patient and/or family Participating family meetings to address patient/family needs Providing spiritual support to patients/families to alleviate suffering and facilitate acceptance of the dying process Utilizing community resources to assist the patient/family to meet goals Coordinating with area clergy and religious leaders to provide spiritual care to hospice patients and families Referring patients and/or families to outside agencies, if necessary Maintain continuity and consistency of care by: Completing spiritual documentation in the patient’s electronic medical record within the prescribed time frame Triaging essential information to other colleagues to communicate spiritual needs as it relates to transitions in patients care In collaboration with Bereavement Services, Spiritual Counselors assist with providing follow-up bereavement calls, required visits and initial assessments per policy Develop and provide educational programs to Coastal Hospice staff and volunteers, local faith groups and community organization, as requested Provide spiritual support to Coastal Hospice staff, as requested and appropriate Master’s degree in divinity or theology or pastoral counseling from an accredited seminary or school of theology preferred Completion of a minimum of one (1) unit of Clinical Pastoral Education (CPE) within prescribed amount of time or equivalent clinical training and experience preferred Demonstrated ability in facilitating individual and group support Sensitivity to the particular needs of those in grief Competitive wages Mileage reimbursement at 100% of the Federal rate, currently $0.70/mile Generous paid time off 403B retirement plan with company match Role Coastal Hospice invites you to become part of the team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions.
