CareBridge Health exists to enable individuals in home and community-based settings to maximize their health, independence, and quality of life through home-care and community based services. The RN Triage Nurse is responsible for triaging acute care needs and issues telephonically. Utilizing department guidelines, completes triage process and applies established criteria to assign members to appropriate care management component. Collaborating with team members and providers to provide appropriate level of care. This role is virtual full-time with required in-person training sessions. Work shift is 4/10 hour shifts, 11 am to 9 pm CST, including full weekend shifts every two weeks. Minimum requirements include an AS in nursing and minimum 2 years acute care clinical experience or equivalent, current unrestricted RN license in applicable state(s). Preferred skills include active RN Compact license, ER/Urgent Care experience, telehealth experience, BS in nursing, EMR systems experience, and experience managing complex care cases. The role involves using nursing process to meet health needs, developing collaborative relationships, documenting clinical information timely and accurately, monitoring inbound calls and tasks, and assigning patients to Nurse Practitioners when necessary.
