POSITION SUMMARY:

Under the direction of the Program Director, the Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has a particularly close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.

JOB DUTIES & RESPONSIBILITIES:

  • Educate individuals and communities on how to use and understand health and social service systems in order to build individual and community capacity.
  • Conduct health promotion and disease prevention education in a manner that matches linguistic and cultural needs of participants or community.
  • Participate in care coordination, case management, and advocacy and lead support groups.
  • Facilitate transportation services and help address barriers to services.
  • Complete all documentation within established timeframes.
  • Participate in design, implementation, and interpretation of individual and community level assessments.
  • Outreach to individuals, families and community groups.
  • Conduct home visits to provide education, assessment, screenings, and social support.
  • Participate in evaluation and research to identify priority needs and issues among individuals and communities.

KNOWLEDGE, SKILLS & ABILITIES:

  • Knowledge of public health principles, healthy life-styles and self- care.
  • Familiarity with the health and social service system.
  • Ability to do home visits and outreach.
  • Experience with usage of EMR/EHRs/Care Management software.
  • Strong organization skills and ability to manage and maintain a personal schedule.
  • Ability to establish priorities and meet deadlines.
  • Excellent oral and written communication skills.
  • Ability to exercise judgment in the application of professional services.
  • Active listening skills and genuine compassion for others.
  • Enjoys collaboration within a team environment and working with people of different skills and experience.
  • Able to maintain clear professional boundaries with clients and coworkers.
  • Demonstrates cultural competency and ability to work with diverse groups of community clients.
  • Comfortable using computer for documentation, communication, and organizing work.
Requirements:

GENERAL REQUIREMENTS:

  • Education/Experience
  1. Lived experience regarding various issues faced by the community served or 1 year of experience in community, public health, or human service work, preferred.
  2. Ability to work flexible hours, including occasional evening shifts.
  3. Multilingual capabilities preferred, but not required.
  4. Long-time resident of the community with good knowledge of relevant resources.

Licensure, Registration, Certification

  1. Valid CA driver’s license and proof of automobile insurance.
  2. Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.

PHYSICAL REQUIREMENTS:

  • Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
  • Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
  • Required to occasionally lift and carry 10-20 pounds.
  • Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
  • Regularly required to sit and use computer in office settings
